And to do this, we are relying on the playbooks of two world-class sources. We are looking at the clinical protocols of Dr. Shim Ching. Trevor: 04:06 When we say retinol speeds up cell turnover, like what is chemically happening in the tissue? Erin: 04:11 So retinol is a derivative of vitamin A, and when you apply it, it actually binds to specific receptors on your skin cells. Okay. And it forces the upregulation of keratinocytes. Basically, it commands the basal layer of your epidermis to manufacture new skin cells at an incredibly accelerated rate. Trevor: 04:30 Oh, so it's pushing the old ones out. Erin: 04:32 Exactly. It forces the older, damaged, pigmented cells up to the surface where they are shed. And at the same time, it penetrates deeper into the dermis to signal your fibroblasts to ramp up collagen production. Okay. Trevor: 04:45 That accelerated shedding makes total sense, but it brings up a major issue for a lot of people. That purge. Yes, the infamous retinol purge. You put it on, and a few days later your face is just red, flaky, and furious. Erin: 04:57 Yeah, it's a very common reaction. Trevor: 04:58 And if chronic inflammation is actually pro-aging, how do we get that cellular upregulation without causing this massive inflammatory response that makes people just want to quit using it? Like the moment you open the bottle, it reacts with the oxygen and the light in your bathroom. It oxidizes, turns brown, and becomes virtually useless. Erin: 08:01 Yeah, it basically turns into hot dog water. Trevor: 08:03 Right. And actually oxidized ascorbic acid can act as a prooxidant, causing the very irritation you're trying to prevent in the first place. Erin: 08:10 Exactly. Which is why Dr. Karem stresses that you have to look for stable advanced derivatives if you want the molecule to survive long enough to even penetrate the skin. Trevor: 08:19 Okay, so stability is key for vitamin C. And because using potent acids in retinol can put the skin in this constant state of low-grade stress, Dr. Karam brings in niacinamide or vitamin B3. Erin: 08:32 Which he also rates in 9 out of 10. Trevor: 09:24 If retinol is the aggressive CEO demanding the factory produce more collagen immediately, peptides are the specific text messages or chemical blueprints handed directly to the workers on the floor. Erin: 09:42 Yeah, because peptides are short chains of amino acids. In nature, when your existing collagen breaks down, it forms specific peptide fragments. Okay. Trevor: 11:25 Sodium hyaluronate. Got it. Erin: 11:27 Yeah. It's a salt derivative of HA with a dramatically lower molecular weight. It is small enough to actually bypass the barrier and hydrate the tissue from within. Trevor: 11:36 Okay. That is a pro tip right there. Erin: 11:38 Yeah. Trevor: 11:38 But at a certain point, the topical routine hits a biological ceiling, right? Erin: 11:42 Yeah, it definitely does. Trevor: 11:43 We've optimized the cells at home, but to induce major structural changes, you need to introduce controlled physical trauma. And we are literally taking an electric device covered in tiny needles and stamping it into the face to create thousands of micro injuries. Erin: 12:52 It sounds completely medieval, doesn't it? Trevor: 12:54 It sounds totally counterintuitive to anti-aging. Like why are we stabbing our faces? Erin: 12:58 Well, it relies entirely on the body's trauma healing cascade. When those needles pierce the dermis, your biology detects a physical breach. Okay. And it instantly floods the area with fibroblasts to lay down fresh collagen and elastin to heal the wound. Trevor: 13:14 Trevor Burrus And Dr. Ching notes, you can amplify this dramatically with PRP, right? Platelet-rich plasma. Erin: 13:20 Yeah, the vampire facial. Trevor: 13:22 Right. Trevor: 15:60 Like adding a little projection to the chin. Erin: 16:01 Right. Or restoring the lips or filling hollow temples. But using them to lift a sagging jawline just results in a puffy, distorted look because you aren't fixing the skin laxity. Trevor: 16:14 But just adding heavy volume. Erin: 16:15 Exactly. You're just weighing the face down. But the more urgent warning surrounds sculpture. Trevor: 16:20 Yes. So a sculptor gets a five or six out of ten from Dr. Ching for deep volume replacement near the bone. But he gives it a stark zero out of ten if a provider injects it superficially right under the skin to try and boost skin collagen. A zero. Yeah. Why is it so dangerous?
